This form must be submitted in order for you to be considered for admission into the professional program.
  Without admittance, you won't be allowed to register for 300- or 400-level classes and can't graduate in the major.
  To get an application and for more information on the due dates please visit the Advisement Center Application web site.
  After you finish with the form, turn it in to the School of Technology advisement center.
