Evaluate the Performance of an Intern

The Student Intern Performance Evaluation pages allow the Experience Provider to evaluate the performance of their interns. The intern's direct supervisor should perform the evaluation following the instructions given below.

You may complete the evaluation online or by completing a hard copy and faxing it back to us.

Please download and complete the PDF version of the Performance Evaluation and fax or mail it back to us (see contact information).
